Forms

Overview

The Forms page on a Participant's profile brings together requests, submissions, and Forms associated with that Participant. If an existing submission is missing, link it to the Participant from the submission itself. See Managing Form Submissions.

Form Submission Requests

This section lists Form Submission Requests for the Participant. Users with the Create Form Submission Requests permission also see Create Request. Available actions depend on permissions rather than a fixed Admin, Manager, or Team Member view.

Form Submissions

This section lists submissions the user has permission to view. Select Fill out Form to start an available Form for the Participant.

Associated Forms

When Scheduling is enabled, Associated Forms lists Forms that Support Workers can complete for this Participant. Select Add Form to associate another Form.
