The Submission Request page hosts the list of submission requests requested by members of your organisation. If you are looking to report an incident, this is not the correct menu. You will need to navigate to the Incidents menu (click here for more information on this). Within this menu you can create a form request and also click into previous requests to see the status and other options in regards to the request.
You can filter this list by the status of the submission request and also by the particular form.
To create a submission request simply click 'Create Request' in the top right corner. To learn more about this process click here for our user guide article.
