Overview

Astalty, with the introduction of the Forms functionality (more on this here) has also introduced the Incidents functionality. The incidents functionality will centre around the reported incidents (Participant and staff) forms with the submitted forms appearing in an incidents register within the software.

How to create an Incident Form

Creating an Incident form will be very similar to how you would create any form within the software. The Astalty forms functionality will release with two incident templates one for the Participant Incidents and one for the Staff Incidents. You can also create an Incident form from scratch and select the usage type within the Create a Form functionality. To learn more about creating a form click here for the user guide article.

Incident Collaborators

You can add collaborators to an incident to allow additional users to view and contribute to the incident record.

Collaborators are users other than the primary assignee who require access to the incident. Once added, collaborators will be able to view and edit the incident, supporting better collaboration and shared responsibility when managing incidents.

This functionality applies to both Participant and Staff Incidents, ensuring consistent access across all incident types.

Adding a Collaborator

  1. Open the incident from the relevant Incidents register.
  2. Click Edit.
  3. Select one or more users in Collaborators.
  4. Click Update Incident.

Collaborators can view and update the incident, change its status, and manage involvees. A collaborator cannot change the assignee or collaborator list unless they are also the assignee or have permission to update that incident type.

Permissions

Access to view and update incidents is controlled by incident permissions, Participant access, assignment, and collaboration.

Anyone can submit an incident as long as they have access to submit the related incident form.

These are the default role permissions. Custom permissions can change them.

Participants IncidentsStaff Incidents
AdminCan view and update all IncidentsCan view and update all Incidents
ManagerCan view and update all IncidentsNo access
Team MemberCan only view Incidents related to their assigned ParticipantsNo access

Collaborators can view and update the incident. Assignees can update an incident they can access, even without general update permission.

Assignment alone does not grant access to a Participant Incident. The assignee still needs permission to view it and any required Participant access, unless they are also a collaborator. For Staff Incidents, an assignee with an Admin, Manager or Team Member role can view the incident. Being listed as involved does not itself grant access.

Archiving still requires the relevant archive permission.
