> View, report, search, and follow up on Participant incidents.

Astalty Premium - This feature is part of the Astalty Premium plan. See What Premium includes.

Use [Participant Incidents](https://app.astalty.com.au/incidents/participant) to report incidents involving Participants and track each report through follow-up and resolution.

When viewing all Participant Incidents, you can view each incident's:

- **Reference**
- **Status**
- **Severity**
- **Category**
- Whether it's **Reportable**
- **Incident Date**
- **Involvees**
- **Site** (for providers with Scheduling turned on)
- **Assignee**
- **Collaborators**
- When it was **Reported**

You can also sort your Incidents by **Status** or **Incident Date**.

The **Severity** column shows the level set during review - **Low**, **Medium**, **High** or **Extreme** - and stays blank until someone sets it. **Extreme** is listed separately from **High**, so the most serious incidents can be picked out of the register on their own. See [Incident Follow Up](https://docs.astalty.com.au/guide/incidents/incident-follow-up#severity) for how severity is set.

> **Site column**
>
> The **Site** column only appears for providers with Scheduling turned on, and shows the site linked to the incident's form submission. Accounts that don't use sites won't see this column.

You can allow Support Workers to have visibility of the Participant Incident Form by updating the Form Settings. See the [Form Settings](https://docs.astalty.com.au/guide/forms/manage-forms/settings) guide for more information on how to do this.

See [Participant Details in the Support Worker App](https://docs.astalty.com.au/guide/scheduling/support-worker-app/participant-details-app-view) for how Incident Forms appear to Support Workers.

## Email notifications

When a Participant incident is reported, configured [Incident Notifications](https://docs.astalty.com.au/guide/notifications/incident-notifications) are sent to the relevant users and groups. The notification email includes the incident reference number in the subject line and a summary of key details - type, occurred time, reporter, category, severity, status, and NDIS reportability - so staff can act without logging in. See [Incident Notifications](https://docs.astalty.com.au/guide/notifications/incident-notifications) for full details on configuring and understanding these emails.

## Incident Follow Up

See [Incident Follow Up](https://docs.astalty.com.au/guide/incidents/incident-follow-up) to update an incident, change its status, and export it.
